Cologuard turns fright into funny with Matthew Lillard

Cologuard Matthew Lilliard

The latest spot from Cologuard swaps jump scares for belly laughs, using classic horror language to make colon cancer screening feel stigma-free and straightforward. Directed by Grayson Whitehurst through Voyager and fronted by Scream alum Matthew Lillard, the film recasts fear of aging, doctors, and the unknown as something you can meet at the door with a grin.

Cut by Sarah Jordan of Leap Year for Blue Hour Studios, the piece aligns with Exact Sciences’ mission: give people an easy, empowering way to take charge of their health.

The setup plays like a midnight chiller: Lillard, alone at home, a knock, a hush, shadows that seem to breathe. But the big “monster” reveal? A delivery driver with a Cologuard kit. The gag lands because the craft leans all the way into genre—creaking sound design, prowling camera, and negative space that turns every corner into suspense—before flipping it to relief and a smile. Watch below:

“We treated the camera like a character, not a bystander,” says Whitehurst. “The moves and framing lead you through tension so the punchline hits harder. By heightening every creak and pause, the reveal of the Cologuard test turns dread into humor—that rhythm was the joy of directing this.”

DP Tom Banks underscores the pivot with cool, desaturated interiors that warm as the package arrives, visually transforming unease into invitation. In post, Jordan fine-tunes the heartbeat: “With comedy and horror, timing is everything. A few frames can change the laugh. Trusting instinct—stretching a silence here, tightening a cut there—made the payoff land exactly right.”
